You know how to trace components? As ground and +12 should be easy to find 

See which terminal is linked to the transistor then that should be your sounder output

 

Does the chip have 555 on it, as with a couple of caps that your timing circuit so your trigger would go to that chip

Why ? Just bin it and by a new bell. surely its cheaper than spending hours fking around with an old bit of tat thats older than the contents of the property your protecting 

Your best option is to trace everything back from the components. I doubt anyone will have any info about it.

Maybe the terminals where labelled on the bells plastics originally.

I can't read the chip well from your images but the IC's possibly an opamp, it's datasheet will help

well, it's not going to be protecting anything, i collect old bell boxes and this is going inside a vented bell box.

I certainly can't recall surface components on  Bell Module I've ever seen...
