Can it be sent down one cat5 cable on different pairs? Or does it have to be a separate cat5 for video and telemetry

No problem sending it down two pairs of the same CAT5.

Can even have both sent down 1 pair if transmitter and camera can do 'up the coax'

Only with passive gear. But yes telem and tp video on the same cat 5/6 is fine. Just be cautious on the wiring of data as it should be daisy chain not star
